Is it free?

Do I have to change my doctor?
No — we work with your existing providers to help coordinate care more effectively.
How do I get started?
Contact us by phone, email, or form. We’ll complete a quick eligibility screen and connect you with a Care Coordinator.
How long does support last?
Services are offered as long as the client remains eligible and engaged — typically long-term until care needs stabilize.
